- All entries will require printed sample(s) of the specific entry to be judged.
- Each entry must be submitted separately.
- Each entry must be accompanied by TWO (2) entry forms: _
- a) One PRINTED FORM, clipped to a printed sample of the entry, and _
- b) One ELECTRONIC form submitted via the Award-specific online form.
- Entry materials will not be returned, and become the property of The User Group
Academy.

media must supply editor names, contact numbers and mail addresses for the specific
print or broadcast media. (Under separate cover, please inquire.)
Completing the Electronic Form
These instructions apply to ALL online registration forms:
- Do NOT insert returns, colons, tabs, html code or other "special" characters
- Supply single, simple responses.
- EVERY FIELD MUST CONTAIN INFORMATION
- If multiple recipients are to win awards, supply names comma separated
- In the event of an Award, shipping addresses are required -- NOT P.O. Boxes
- Please use proper upper and lower case (as opposed to all caps) -- this data
will generate labeling for the Awards Show, the web site, and other display collateral
materials.
Citations of Participation will be awarded TO ALL ENTRIES whether they win or
not. It is very important that your information be correct and current, as it will
be used to produce the final awards and citations.
BEFORE you click "SUBMIT" we recommend you PRINT the form, and/or SAVE
any completed forms for your records, and to supply as the required attachment with
your mailed entry.
